Benefits

All newly hired employees or current employees who have made a transition to a full time or part time status are eligible for benefits 30 days after their hire date or status change. Full time employees are eligible for all benefits and accrue two weeks of vacation per year and Part time employees are eligible for 401(k), vision insurance, vacation (1 week per year), sick leave (at half accrual) and a flexible spending program.

HOLIDAYS

All Full Time and Part Time employees shall be eligible for seven (7) holidays.

*Full Time employees are entitled to eight (8) hours each
*Part Time employees are entitled to four (4) hours each

Observed holidays are as follows:

  • New Year's Day
  • Memorial Day
  • Independence Day
  • Labor Day
  • Thanksgiving Day
  • Christmas Day
  • Easter Sunday

LIFE INSURANCE

Basic life insurance is automatically given to all Full Time employees at no charge.  Full Time employees may choose to enroll in additional supplemental life insurance, which may be purchased.

  • Services provided by HMA Hospital - 100%
  • One (1) $250 per admission per diagnosis co-pay

PRESCRIPTIONS

Prescriptions are not covered under health insurance.  However, by enrolling in the health insurance plan, you are automatically enrolled under CVS/Caremark Prescription Care Program and have a separate deductible of $50 per person ($100 family).

DENTAL INSURANCE PLAN


  • $50 Deductible ($150 Family Deductible)
  • Preventative Services - 100%
  • Oral exams once every six (6) months
  • Prophylaxis (cleaning of teeth) once every six (6) months
  • Others (refer to Summary Plan Descriptions)
Basic Services After Deduction, 80% paid:
  • Tooth Extractions
  • Analgesia and local anesthesia
  • Oral Surgery (including removal of impacted teeth)
  • Repair or re-cementing of crowns or inlays
Major Services covered at 50% co-pay (UCR):
  • Inlays, crowns
  • Initial installation of fixed bridge work
  • Installation of dentures to cover teeth extracted while under this plan
