Preparing for Discharge
Normal discharge time is prior to 4:00pm. There will be no room and board charge on the day of discharge. However, charges may be incurred for medical supplies, medications and medical testing. Your physician will let you know when you are ready for discharge.
Your nurse will give you discharge instructions including diet, activity, medications, special instructions and doctor's appointments. If a special diet has been prescribed, the dietitian or nurse will review the diet with you and answer questions before your discharge. If you have questions regarding your diet after discharge, please contact the hospital dietitian at (919) 497-8445.
Checking Out
As you are planning to leave, use this checklist to be sure you haven't forgotten anything. The nurse will discuss the following with you:
- Do you have your Admissions packet with the following included?:
- Your special diet, if your doctor has prescribed one?
- A copy of your discharge instructions?
- Information on future appointments?
- Do you have all the equipment needed for your care at home?
- Have you received patient education materials?
- Have you checked your room, closet, drawers and bathroom?
- Have you taken care of your financial affairs with the Business Office?
Upon discharge, the nursing staff or an Auxiliary member will escort you to where your transportation is waiting.
Case Management / Discharge Planning
The Case Management / Discharge Planning Department provides staff to assist the patient and family in arranging for post-hospital care. The department coordinates between the patient, the family and the physician for optimal quality care while assisting with any problems or concerns during your hospital stay.
The services are FREE and confidential and may include referral to:

- Home Meal Services
- Medical Equipment Suppliers
- Support Groups
- Transportation Services (Stretcher Van, Ambulance, Advanced Life Support Air Ambulance)

- Diabetes Self Management and Training
- Medical Nutrition Therapy
- Acute Rehabilitation Facilities
- Adult Daycare
- Alcohol/Drug Rehabilitation
- Outpatient Therapy
- Financial/Disability Assistance
- Adult/Child Protective Services
- Advance Directives Assistance
If you or a family member would like to talk with a Case Management/Discharge Planner, you may contact this valuable resource by:
- asking your Doctor or Nurse to call for you;
- calling the office directly at (919) 497-8495, Monday through Friday; or
- stopping by the office (ask any staff member for directions).
Office hours are by appointment Monday through Friday.
CONCERNS ABOUT SAFETY OR QUALITY OF CARE
