When You Arrive

Admissions

Your hospital stay starts at Admitting where you will be asked for:

  • Name, Address, Date of Birth and Social Security Number
  • Health Insurance Card, Photo ID
  • Person responsible for financial arrangements
  • Person responsible for making healthcare decisions, if you are not able

If you are a MED-KEY cardholder, present your card to the Admissions Coordinator. (Copies of your insurance card will be made for our records.) If insurance such as Medicare, Medicaid, or Commercial Insurance does not cover you, you will be asked for a cash deposit.  At this time you will be given information regarding "Advance Directives" prepared by the state of North Carolina (Living Will and Health Care Surrogate).  During your stay, if you have an questions regarding "Advance Directives", please contact your nurse.

Your Admissions Coordinator will:

  1. Assist you in completing your Admission information.

  2. Work closely with you to handle the details of your individualized care.

  3. Answer any questions you might have about charges and financial arrangements for insurance coverage.


Emergency Admissions

Emergency Admissions are made any hour of the day or night directly through the Emergency Department.


Consent for Treatment

The patient or nearest relative must sign an informed consent for treatment and/or surgery. A parent or legal guardian must sign an informed consent for treatment of a minor. Additional forms may need to be completed at this time for different insurance agencies.
